ESTABLISHING YOUR PERSONAL FILE
Welcome to your new CMP National Registry
Personal File.
Every member of the National Registry will be given their own
personal file.
The personal file will be set up at the time of registration
which will be done immediately upon successful completion of a CMP Concussion
Management Certification Course. We strongly recommend that you
complete the registration at the time you take the course so that
your facilitator may help you if needed.
In order to complete your CMP Certification this form must be
validated by the Facilitator who conducted your CMP Concussion
Management Certification Course. Therefore, when you have completed
filling out the information below, send it by email to your
Facilitator who will then validate it and you will be immediately
added to the CMP National Registry.
The information in the sections marked with a (*) will be locked
in permanently. However, there are some sections that you can alter
whenever you log back in after you are registered.
SPECIAL SECTIONS
Optional Resume Section
An optional section of the member registration form has been set
up for members to add other qualifications, certificates and/or
experience that would provide more information about their
particular qualifications with respect to coaching student-athletes.
This section can be updated at any time after the membership is validated.
Changing Email and Password
You may also change your email address and password if you
choose. It is important to make sure that your email address is
accurate since this is where the "continuing education"
material will be sent. If you do not acknowledge receipt of the
continuing education information you will be removed from the
National Registry.
Forwarding Information To Third Party
There is a section at the bottom of the form that allows you to
forward the certificate information to another email address. This
will be necessary if someone wishes to confirm your qualifications
as a CMP Certified Member. All of the information that is entered
on your Personal File will be sent to that email address.
MAINTAINING YOUR MEMBERSHIP IN GOOD STANDING
In order to maintain your membership in good standing you MUST
acknowledge receiving and reading the special information newsletters and bulletins that
are part of the CMPI Continuing Education Program.
IMPORTANT NOTICE:
TO ALL MEMBERS OF THE CMP NATIONAL REGISTRY
You must renew your membership in the CMP National Registry by
visiting the Continuing Education Centre at least once every six
months.

HERE and go to the Continuing Education Centre where you can
read the latest bulletin updates about concussion management.
If you have not renewed your membership within any six month
period, your name will be taken off the CMP National Registry until
you go back and renew.
This is being done to ensure that all members of the CMP National
Registry have the same basic qualifications with respect to
concussion management, regardless of where they are located in
Canada and the United States. It is this consistency of
qualifications that is the greatest benefit of the CMP
Student-Athlete Concussion Management Program. |

Information Email Link
Three or four times per year you will receive an email containing
a link that you must go to in order to find the newest information
that has been sent out. When you link to that page your National
Registry Membership will automatically be validated and
renewed.
Re-Activating Membership If Necessary
If you fail to acknowledge one of the continuing education
bulletins, your National Registry Membership will be suspended and
your password will no longer permit you to access your Personal
File.
In order to "re-activate" your membership in good
standing all you have to do is visit the Continuing Education
Section and read the information on that page. Then simply fill out
the "declaration" section to verify that you have read the
material. Your National Registry membership will then be
automatically reinstated.
FORWARDING INFORMATION
If you would like to forward this information somewhere to confirm your
National Registry Membership just fill in the email address in the
space at the bottom of the form and press the SEND EMAIL
button.
PRINTING INFORMATION
You can also print a copy of this certification for anyone who is
requesting a copy of the confirmation by emailing it to yourself and
printing it out..
UPDATING INFORMATION AND LOGGING OFF
When you press the UPDATE RECORD button you will be automatically
logged off. |
