IMPORTANT REMINDER:
You must renew your membership in the CMP National Registry
by visiting the Continuing Education Centre at least once
every six to twelve months.
Take a moment right now to CLICK
HERE and go to the Continuing Education Centre where
you can read the latest bulletin updates about concussion
management and you can renew your membership by filling in
your user name and password.
If you have not renewed your membership within any twelve month period, your name will be taken off the CMP National
Registry until you go back and renew.
This is being done to ensure that all members of the CMP
National Registry have the same basic qualifications with
respect to concussion management, regardless of where they are
located in Canada and the United States. It is this
consistency of qualifications that is the greatest benefit of
the CMP Student-Athlete Concussion Management Program. |

To Change Your Email and/or Password
You may change your email and/or password at any time by entering
the new email and/or password in the box below. Your current email
and password that you used to enter the site will no longer be valid
once you enter and confirm the changes.
To Forward Your National Registry Confirmation:
If you would like to forward this information to confirm your
National Registry Membership just fill in the email address in the
space at the bottom of the form and press the SEND EMAIL
button.
Updating Your Qualifications & Experience Section:
You may amend your Qualifications & Experience section at any
time. It is advised that you keep this section up to date and use it
when applying for coaching positions or for any other positive
purpose for which it can be put to use.

When you have completed the updating of your Personal File press
the UPDATE RECORD button and you will be automatically logged off. |
